Call the office to schedule

New patient appointments are scheduled by phone at 253-839-3622. When you call, the office will ask a few questions to help determine the right appointment type and to confirm whether your insurance can be accepted.

Complete your forms online

After you have called and scheduled your appointment, please complete the new patient paperwork online so the office has your information in advance and your check-in process is easier on appointment day.

Arrive 10 minutes early

Please arrive about 10 minutes early so there is time to review your paperwork before your appointment. For a quick checklist before your visit, see the What to bring section below.
